Center for Hospice Care Receives 5-Star Rating

Center for Hospice Care (CHC) is proud to announce that we have earned an overall 5-star rating in the Centers for Medicare & Medicaid Services (CMS) Hospice Caregiver Experience survey, placing CHC in the top 1% of the nation’s hospice providers based on feedback from families and caregivers. Only 417 of the country’s more than 5,000 hospice providers received a 5-star rating. CHC is the only St. Joseph County-based provider to receive this rating.

The CMS Hospice Caregiver Experience survey measures the experiences of family members and caregivers of loved ones who received hospice care. Ratings are based on caregiver feedback on communication, emotional and spiritual support, symptom management, care coordination and overall satisfaction with the hospice experience.

“A 5-star rating is especially meaningful because it reflects the voices of the families we serve,” said John Mastrojohn III, president and CEO of Center for Hospice Care. “Their trust inspires us every day, and we remain committed to delivering compassionate, person-centered care that exceeds expectations and honors each individual’s journey.”

The CMS 5-star rating helps patients and families compare hospice providers and make informed end-of-life care decisions. The rating is based on standardized surveys completed by family caregivers after a patient’s death. Their responses reflect the quality of care and support provided throughout the hospice journey.

Milton Adult Day Services and Center for Hospice Care Honored for Dementia Care

Milton Adult Day Services (MADS) and Center for Hospice Care (CHC) were recently awarded the Becky Zaseck Professionals of the Year award at REAL Services’ (REAL) 30th Age of Excellence Awards. For 60 years, REAL has supported older adults and those facing financial challenges to live with independence, dignity and resilience.

CHC and MADS were recognized for transforming dementia care through innovation, compassion and a commitment to person-centered support. According to MADS Director Sarah Youngs, “It was an honor for our organization to receive the Becky Zaseck Professional of the Year Award. This recognition reflects the dedication, compassion and hard work of our entire team, who strive every single day to make a meaningful impact in the lives of individuals living with dementia and the families who love them.”

MADS, part of Milton Village, has been a program of CHC since 2016. Opened in 2022, Milton Village is a single destination for anyone dealing with Alzheimer’s or other neurological conditions. Guests at Milton Village enjoy a safe environment where caring, warmth and compassion encourage each one to flourish. Meanwhile, their family members or caregivers can benefit from the services of Alzheimer’s & Dementia Services of Northern Indiana.

Youngs noted, “Milton Adult Day Services plays an important role in helping people age in place and remain connected to their lifestyle, sense of purpose and overall health while receiving the support they need. Equally important is the opportunity for social inclusion and meaningful engagement, something that every person needs regardless of age.”
