Job Responsibilities/Expectations:
Collects, records and assesses pertinent data from available resources for admission assessment in accordance with Campagna Academy, Inc. Oasis Program policies and procedures and in a timely fashion. Such data will include, but is not limited to:
Identifies, interprets and documents information on an on-going basis, collected in nursing interview, observation, physical and assessment and diagnostic data and confers with other health care professionals as appropriate.
Synthesizes knowledge gained from a variety of resources and develops age appropriate interventions for the client’s plan of care.
Evaluates data obtained by others by reviewing client’s treatment plan and multidisciplinary assessment for assigned clients.
Assesses clients with more complex needs using appropriate written resources and other health care professionals; documents findings in the client’s record.
Identifies problems with unit systems, communication patterns and unit resources that impact on client care and suggests possible solutions to Program Director. Assumes an active role in identifying unit educational, equipment and supply needs.
Identifies potential client care problems, abrupt changes or impending instability in the client’s condition and exercises leadership to intervene appropriately and prevent adverse client outcomes:
Identifies health education needs of the client/family that need to be addressed before discharge. Documents needs in treatment plan and develops plan to meet those needs.
Implements age appropriate teaching interventions to meet health education needs of client/family and documents appropriately in the client’s record.
Actively involves the family and/or client in planning care and documents outcome in the client’s record.
Participates in planning and modifying the client’s plan of care.
Incorporates information from client, family and health care professionals for revising the client’s plan of care.
Demonstrates courteous, cooperative and respectful behavior when working with clients, families, physicians, visitors, and The Cornerstone, Odyssey, and Ark employees. Maintains client confidentiality at all times.
Protects clients, visitors and staff from environmental hazards by adhering to the safety and infection control standards.
Acts as a client advocate, demonstrates knowledge of client rights and responsibilities, protects client privacy and confidentiality.
Implements client care based on established care plans, cliential policy/procedure and unit standards of care, incorporating the client’s age specific and cultural needs as appropriate.
Reassesses needs and problems based on changes in client status, changes in treatment, or identified client need.
Evaluates the effectiveness of performance improvement and unit system changes and provides feedback to the Program Director to support positive change.
Engages in on-going self-appraisal in relation to job performance and personal and professional goals.
Participates in continuing education or in-service programs to increase clinical competence and to meet professional needs and goals. Reports information obtained from continuing education endeavors to unit staff.
Demonstrates consistent acceptance of professional accountability for own practice shown by follow-through on client and organizational concerns.
Participates in professional, constructive peer feedback activities as opportunities arise and as requested by the Program Director.
Serves as a role model for peers and others in effective use of communication, teaching, and leadership skills.
Supports Campagna Academy by providing thorough implementation and adherence to the Standards of Nursing Practice and Standards of Client Care.
Supports changes in staffing/scheduling when unit client care needs require.
Regularly contributes to nursing functions through active participation on committees and attendance at designated meetings.
Participates in performance improvement activities on a regular basis.
Assists in the development and implementation of unit standards of care as directed.
Able to assume nurse role.
Recognizes need for support and seeks appropriate assistance.
Demonstrates initiative in compliance with required in-services. Attends 100% of required in-services as scheduled.
Facilitates approved medication dispensation course to qualify Youth Care Specialist.
Maintains a professional appearance by dressing appropriately and by being well groomed.
Assess injuries that staff occur while on the job and make medical recommendations based on the assessment.
Attend all training required by State and Agency requirements (Mandatory) which shall include: HIPAA, Sexual Harassment, Universal Precautions, Fire/Safety, Cultural Diversity, Nutrition/Sanitation and complete their annual total hours of training and take the annual TB test prior to the Evaluation process.
